As global aging and sustainable development are both on the rise, this study constructs a community elderly care model that integrates ecological benefits and cognitive health. Using mixed methods (questionnaires N = 487, interviews N = 38, and community observations), it examines the relationship between community environments, green elderly care awareness, and service demands. Results show a moderate awareness level (M = 3.21, SD = 0.87), with urban residents scoring higher (3.45 versus rural 2.92). Over 70% of elderly people demand cultural activities and fitness facilities, while garbage sorting guidance shows low satisfaction (M = 2.78). Notably, community sustainability correlates positively with cognitive memory needs (β = 0.41, p < 0.01). This study offers strategies for optimizing environment design and intelligent service delivery to promote sustainable, cognitively supportive elderly care through information-driven approaches.
